Thank you for your interest in supporting the Indiana Pain Initiative and our efforts to improve the lives of the one in four Hoosiers who are in pain. Your generosity will enable us to continue our efforts to ease the pain and suffering of our fellow Hoosiers.

You can donate a gift on your behalf or in honor or in memory of someone whose life has been touched by pain. There are two easy ways to donate:

1. Please print off and complete the donation form and mail it along with a check or money order payable to Indiana Pain Initiative, 3921 N Meridian St Ste225, Indianapolis, IN 46208. Click here to download donation form.

To make a donation, simply complete the form linked and mail it along with a check of money to the address at the bottom of the page or fax it to the INPI office with your credit card information. You can donate a gift in honor or in memory of someone whose life has been touched by pain.

The Indiana Pain Initiative is a program of the Indiana Hospice and Palliative Care Organization, Inc. a nonprofit 501(C3) tax exempt organization as defined by the IRS and as such most donations are tax deductible.
