Hospice RN Case Manager Full Time Posistion available in Indianapolis, IN. The RN will provide hands on clinical care and manage a case load of patients and provide skilled nursing assessment, planning and care in order to maximize the comfort and health of patients and families.
Requirements: Graduate of an accredited Nursing Program, BSN preferred. Minimum 2 years experience, preferably one year in an acute setting, and homecare or hospice experience preferred. Demonstrated ability to assess and respond to the needs of patients and families in varied settings. Demonstrated ability to work independently and cooperatively as a team member. Ability to communicate effectively. Valid RN license in IN. Current CPR certification. Current driver's license. Reliable transportation and current auto insurance. CHPN certification preferred.
